In summary after reading lots of posts:

The only real "casualty" seems to be the Magnavox box from Wal-Mart that overheats easily and people have had DOA's as well as quick deaths, and leads them to drill holes in the case.

The only other problems seem to be random lockups requiring reboots of boxes with digitial closed captioning or particularily bad signals (regardless of CC), or the infamous left audio sound bug affecting all LG chipset boxes.
