...Hardcore or Insane - other than for achievements? Is there anything additional when you complete it on a difficulty other than Casual?

While this is definately the most important thing you achieve, you also earn a special gamer pic for finishing all missions on Insane.

The most fun I had playing the Campaign was on insane, playing on-line co-op. I met several nice people that are still on my friends list.

Especially if you can do it alone with the stupid team AI , it's much easier with another skilled Co-Op player IMO (and I am only about half way on Insane by myself) .

Unless you're a "hardcore" gamer, I'd go ahead and play it on "Casual" to save yourself any frustration. That way you'll go through the experience and enjoy it, instead of hitting some, in my opinion, poorly designed areas that will become an exercise of frustration.

I loved playing through it on insane. I did it all in one-player mode. Except the final boss battle, which I could not beat no matter how many times I tried. So, I'm stuck without that special picture.

But I'm still awesome.

just run to back up the stairs so you can just barely see him and use the sniper rifle on his head...

Yep, the only part I was unable to do on Insane was the final boss, actually ticked me off when I completed that part on co-op with someone in less than 30 minutes after trying frutlessly for like 2 hours by myself and easily dying at least 60-70 times. I had lost count way back when :) Doing it on Insane especialyy by myself was extremely satisfying and really the only part that almost had me ripping my hair out(besides the end of course) was when you are on the second floor of the mansion right after the cutscene and the enemies storm the front door of the place. Now that part was a real bitch to say the least.

Indeed, for the awesomeness.....and the achievements. Those are the two reasons I did it, and I did it alone. A certain few parts had me swearing up a storm ala Call of Duty 4 Veteran, but its doable and fun/challenging.

I actually thought the final boss was cake if you had the right strategy. Save up all your torque bow arrows throughout the level. Load RAMM with perfect reload torque bows into the head, and when he gets near the wall blind toss every grenade you have. He went down before he even got to the wall the first time.

I beat the game myself on Insane. There were a few difficult parts but overall not bad. The first was underground, at the end before you plant the resonator you have to storm that structure with the Theron Guards. Then in act 4 when you have to escape the mansion and head towards the vehicle. That was hard until I figured out all you need to do is run past the enemies, although it helps to kill the two Boomers. And finally the end, when you have to climb on top of the train and use the chainguns, that part gave me all kinds of trouble. The last boss was fairly easy (or I got lucky). I used the Torque Bow/Sniper combo until he got close then blind tossed a few grenades, the last one killed him.

When I replayed it on insane I actually had an easier time with RAAM then when I played it on casual. On casual I eventually had to bail and get a co-op partner to beat him. However since I had beat the game once I was more relaxed on insane. I mean the worst that could happen is I could not beat him, but I have already seen the ending so what does it matter. Then Boom I beat him on my 3-4 try. I was so happy!

On the other hand I did have issues with two areas on insane that I did not on casual. One was on top of the train taking out the flying guys. For some reason I had a LOT of trouble with this. But the most hard was trying to get through the cartroom with the exploding guys and a useless partner. Though eventually he got "put down" in front of the door to the room I was in and that helped a lot.
