Just got GTA4 and it looks like crap on my Sony XBR4 52" monitor. Is there some setting I need to get the scaling or resolution correct?

Go to settings (GT4) and choose 1080i instead of 480i or 480p.

GTA4 just isn't that good looking of a game, regardless of platform. The PS3 version had the additional negative of rendering at 640p (if memory serves) and then scaling to your desired output resolution. If the game looks rather aliased, and everything at a distance looks rather blurry, then this is normal/intended.

The PC version looks promising, visually. I'm able to look past the 'crappy' aliased graphics though, the game is so fun =D
