I just purchased these seats but I do not have the model number for them. I need the model number as one of the switches is cut in half and I need a new one, or find out what type of plug it is so I can wire a new one on. It looks like a RJ9 connector.

The switch is for the power recline.

The model number of the Power Control box is JLDP.

These are 45001 - they have been discontinued.

Yeah, I figured they would be discontinued. I got two of the chairs pictured, and one of the bigger ones that has both arms on it. You wouldnt know the model number of the bigger one with both arms do you?

If it has two staright arms than it should be 45001-3901.

If it has Left Facing Straight armrest and Right Facing Wedge armrest than it is 45001-5341

If it has Right Facing Straight armrest and Left Facing Wedge armrest than it is 45001-5241

I think I got a good deal on them.

I got two of the style pictured above, and one with two arms, they all 3 have buttkickers, and are power, all for $275.00

Wow, just got the replacement cable for free...

Well they sent me a power module instead of a new cable...looks like I need to call back.

Big props to Berkline customer service, they have always been very helpful when I call.
