I'm curious about this too. Microsoft may or may not still offer this for free. When I tried calling the support line, I just got an automated message directing me to download an online form. I found the form and faxed it in, but it has been 2 weeks now and I have not received my data migration kit.

Has anyone successfully ordered the data migration kit from Microsoft recently?

I just picked one up a week ago from Microsoft. My reasons were different (My Elite suffered the E74 error and I was still able to return it to Sams so I bought another one and migrated all the data from my original Hard Drive to the new one) but the migration went smoothly.

Like you I had difficulty finding out how to get one so I called their support at (800) 4MY-XBOX and they shipped the kit out for $12 and some change including shipping. It came in about 3 or 4 days. I opened a ticket on my bad unit so I would open a ticket on the unit you are migrating from and you will get yours.

Oh and the other nice thing is you can now transfer your licenses for games or add on's you've purchased to your new harddrive.
