I'm considering locating a MBM-12 behind my sofa and would have to run wiring under the carpet. When I did this with my side surrounds, I found some flat wiring that worked well.
However the MBM wiring should probably need to be a shielded coax with RCA plugs on the ends. Has anyone seen a flat version of this??

No, but I just used a thin RCA cable from Radio Shack that is run under the carpet in front of my fireplace.
I can't find it on their website, but it was $7.99 I believe. It was with the video cables section. It works great.
How long is it?
I'm needing something between 25' and 30'.

Coax design cable in general is not as resistant to being stepped on repeatedly as is flat twin-lead. Name brand microphone cable specifically designed for on-stage use has the greatest impact resistance, but is not small in diameter.
Your best bet here may be the thinnest RG-59 -- not RG-6 -- coax you can find at the x-Mart nearest you. Most cheap RG-59 has a hard dielectric that may give you the step-on resistance you need.
If there is another path, like along the baseboard, up and over a door, etc. I'd use that and good quality coax even though the run is longer.

It's 25', and it's so thin that isn't isn't any taller than the tackstrip that it rests next to. I have it right between where the pad ends and the tack strip, all under the carpet.

You can get nearly perfectly flat wire for under your rug - its no thicker than paper, but it is expensive.
