Panamorph Mounting Bracket

I have the UH480 Lens with the automatic sled, and can't find out where to buy the mounting bracket that affixes the two properly. any suggestions?

Try writing Panamorph directly:

Is this what you're looking for? If so and you're still looking send me a PM. I had a spare one of these in my closet, brand new - had to cut open the packing bag to pull it out to take a picture.

That looks like an older plate with the support chains. Still, it should work as long as it is for the same projector model. What was it purchased for? Hole patterns are sometimes projector specific.
