Does anyone know where I can purchase one of these? This is mainly for another DIY lens try... Has anyone had any significant success with this yet?

What magnification is that? You need 1.33x for video and that looks like a film lens which (today anyway) is 2x. There was older film lenses that were anything from 1.42x and up.
