I had a 65" Panasonic delivered that had at least a dozen long cracks in it - some up to 18" long. All of them were internal - the outer glass was not cracked at all.

In-wall speaker wire is fire rated. Would be prudent to use for insurance purposes so that if you ever had a fire it couldn't be blamed on improper wire.

Many amplifiers distort heavily when turned up all the way. I believe this called "clipping". You fed this distorted, clipped signal to your speakers and cooked them.
