With a DD 2.0 input signal, that would make sense, otherwise if a 5.1 signal with a 5.1 setup, AFAIK, "Dolby Digital" and "Dolby Digital + Dolby Surround" should be no different.

Ah .. misunderstood. I don't use HDMI(ARC) but once you set it up, AFAIK, it would only apply when using the HDMI(ARC) feature, so you shouldn't have to manually change it each time.

Most likely yes, as most (if not all) must be set to "TV" or "external speakers"; however, you normally would not want TV speakers active at the same time as the surround speakers.
