I'd place the rear surrounds closer together rather than on the same wall as the (side) surrounds. And I'd place the surrounds ~2' above ear-level when seated (or ~55 inches from the floor).

Yup, those are two separate configurations according to your original post.

IIRC, the TV should be placed so that your eyes hit the screen roughly 1/3 up from the bottom of the screen.
