As was said above the voice control will only respond when you say "xbox" first..... like "xbox play" "xbox pause" xbox eject" etc.
The accidental click when using gesture control is remedied by making you "hold" on an onscreen button for a few seconds (you can see in the dashboard videos when the circle runs around your hand icon). It will only click that button after the circle has run its course..... should solve that problem pretty nicely.
