Your best bet is to find the thread with the list of ISF Calibrators and their locations/contacts and ask them yourself. The cost can vary....

Regarding using a disc, you can use DVE HD Basics, AVIA II, Speers & Munsil, etc... these are good to get a pretty solid PQ but are not going to go as in-depth as a trained and certified ISF Calibrator is capable of going. The discs will not provide you with the same level of PQ a professional can. The flip-side is that using the discs is much, much cheaper and yields what most people consider a perfectly pleasing image.

By all means, adjust your display's black level, contrast, color & tint tilizing a calibration disc first. For some, that is enough.

For others who want to maximize their display, you'll need to do a full calibration, or hire a pro.

Price for a pro calibration depends on the display, whether it has 3D or not, componants (AVR, gaming system, BD player, sat/cable box, etc), number of inputs and modes you want calibrated, and the selected calibrator's experience, expertise and equipment utilized.
