Since it is ripped with just the "Main Movie" the mpls is numbered 0000...

It is easier if the rip is on the root folder of a drive (less typing).
So, lets say you put it on the root of d:\ drive...
open eac3to in command line and type:
that will give you a list of playlists (yes, this is important if you have a disk with multiple playlists)...and let's say this one only has one (playlist #1 is the main one and only one)...
type: eac3to d:\terminator\ 1) 2: d:\terminator.mkv 3: d:\terminator.wav 4: d:\terminator.ac3
This example supposes that track #2 is the video, #3 is the PCM audio, and #4 is the secondary audio or commentary track or whatever.
After its done, open up mkvmerge and just add the desired tracks and select a destination location and create the file with .mkv extension.
