I use DVDfab Bluray Ripper mkv.remux to get the subtitles in an IDX file and a MKV. Then I use MKVtoolnix remux the IDX subs back into the MKV. It worked well for problem discs like Avatar and the Star Wars saga.
The problem is if I Handbrake these to get them into H264 the audio has a couple hiccups.
