I conduct usability studies for mobile apps. Basically, people come in and use the software we're building while we watch to see where we've gone wrong. I need to be able to:

1. Mirror the phone screen (Galaxy Nexus) on an external monitor (so the audience can watch the user interact with the app from another room)
2. Output audio (what the user is saying) to the same monitor from an external mic
3. Record the whole thing for review later

I have an MHL converter for the Galaxy Nexus and a 100ft HDMI cable which allows me to mirror the screen on the monitor, but that's as far as I've gotten. Looking for suggestions for display and record at the same time, as well as integrating the audio bit.
