I think the fan draws air from a hole in the bottom, then hot air is pushed out the top rear. The HDD mounting bracket blocks direct airflow and air is pushed out both ends of the HDD mount. Picture showed they just removed the fan from a standard HSF. Then air just go around the sides of the HS not providing much cooling.
For any Sandybridge/Ivybridge CPU to cool, even 35W versions, an active HSF must be installed, probably a very low profile ones. But most low profile HSF designs need air from top. So the airflow in this case is a mess.
