That's horrible. I haven't been logging any stats, so I don't know if I have packet loss as well. Probably a good idea to check since I get lag as much as the next guy, but not as much as you!
But if did have packet loss I would investigate by isolating the problem.
First you have to clear your devices to make sure it's not your PC, router, or modem causing the problems. The PC could have a firewall causing the problems, but if you get the same results on other computers then you can assume it's not the PC.
Run without a router.
Swap the modem with a loaner to eliminate the modem as the source of the problem.
When you've cleared each device, then the next step will monitoring your connection for extended periods of time to find the source of the loss. Probably a good idea to have your ISP roll a truck at this point and do the monitoring for you since you've already trouble shot the basic stuff, and you know it won't be a problem with your setup. There are also web based services that will do that type of monitoring for free. I think you can do it yourself with a program like Wireshark, although I never used the app.
