Yeah, the key is really situational awareness. Traps are more dangerous than enemies. You can't hurry. There's no benefit to killing enemies, so pick your fights. You can get health back. There is a "damsel" somewhere on each level (it can actually be set to be a girl, guy or dog, but is referred to as a damsel). If you pick her up and carry her to the level exit, you get one health back. Wooden crates give bombs, ropes and items. It's always worth burning 1-2 ropes/bombs to reach a wooden crate, as they give 3 ropes or bombs.
Between the two of us, we've probably had 200+ runs on it, although some of those might have only been a couple of minutes long. When we started, just reaching the second world seemed like a major victory. Now we can consistently reach the 4th world, though we still haven't actually beat the game. There's a bunch of hidden/surprising stuff. I'd avoid looking anything up for a bit, just for the possibility of randomly discovering some of the really neat stuff.
