These claims are usually greatly exaggerated. It does happen from time to time but it is usually pretty easy to fix. Most of the time this is caused by the customer either forgetting/refusing to send the receivers back when they are supposed to, however sometimes it is DirecTVs fault for not correctly getting the returned receivers checked back in to their system. If you make sure to keep the tracking information for the receivers that are sent back, then you can show that to DirecTV as proof that they were returned and they are usually pretty quick to refund the money to your card.

Whole Home DVR is great. I have had it for a couple years now (I beta tested it before it was widely available) and couldn't go without it anymore.
DirecTV will try to re-use existing cabling if possible. However they too must make sure the splitters in your home are rated high enough (3Ghz I believe) and the cable is good enough (they prefer RG6 with solid copper core, but I believe they can still use RG59 if it is in good shape). Otherwise they will rewire the house. Free install is running the wire on the outside of the house and drilling in through the exterior wall. If you want wall fishes to interior walls they will charge extra.
Old splitters will not "burn out" they will just block the signal so it is unusable.
As far as your questions about Dish's price lock-in, the way I understand it is they have promised not to raise package prices on you, but they can still raise the fees that they charge for DVR/receiver rental, and stuff like that. So they can theoretically still raise your bill.
