Yes, I had an "Uncharted: Drake's Fortune" disc come from Game Fly with a huge scratch that rendered the disc unplayable. Other than that the worst I've had is fingerprints that just needed to be cleaned off.

Yup, Boiling. That particular fix was most common for combo-discs. Particulate from fabbing the DVD-9 side (which was done after the HD DVD side?) would stick to the HD side. Apparently the final cleaning at some of the replication facilities wasn't as thorough as others. We'll never see that problem on Blu as it has to be super clean before the anti-scratch goes on.
