Question How To Connect Riser Wiring to Wall?

I've been searching for several hours and I still haven't found a good explanation for this.

What is the best practice method for connecting the line voltage and low voltage wires to the riser from the wall? Are people just spanning the small gap with bare wire or are they using flexible conduit? How are you maintaining sound isolation at the wall connection?

Small gap 1 inch or less = bare Romex between the wall and the riser framing tucked below the top edge of the riser framing out of sight and out of reach. The carpet and any molding will cover the gap

Larger gap add some blocking to protect the wire.
