I use SageTV with their HD Theater extenders to distribute video throught my house. I have a "server" PC running SageTV in the utility room that has an HD HomeRun (OTA) and Hauppauge HD-PVR (Dish HD) tuners that record my TV. I've also got all my DVDs and BDs stored on that and my NAS. The SageTV software does the recording an media management activities and provides the interface to the HD Theater extenders that I have in my HT and my living room.

So I can access all my recorded TV/DVDs/BDs/videos in those rooms.

For music I have Squeezebox and Roku Soundbridge.

I'm really not a fan of a 100% do it all music/video solution because they almost always require the TV be on, and if I have the TV on, I'm almost never listening to music. And likewise if I'm listening to music it's either because I don't have the TV on or I don't have a TV in that location at all.
