Here is my planned setup:

* Centralized HDBaseT switcher for video
* Centralized multi-zone audio distribution system
* TV in each room
* In-ceiling/wall speakers in each room
* In-wall volume control (with IR) for in-ceiling/wall speakers

The speakers are wired to the centralized audio distribution system (and going through the volume control). I also want to use these speakers for the TV (instead of the speakers built into the TV or a soundbar). To do this, in the central location I could wire the audio output of the video to the audio distribution system. However, I am afraid that due to different paths the audio and video could be out of sync. Is that a real concern? If so how would I prevent that?

Depending on models yes you can have a delay. It's not HDBaseT but Video Storm has an IP solution and a suite of video distribution and audio switches that will eliminate the sync issue.

In my experience this hasn't been an issue. I've installed everything from 4x4 HDBT and 4x4 audio matrices together, to 20x20 HDBT and 16x32 audio matrices together.
