You can't convert HDMI to coax, you should have had two HDMI cables added to the pre-wiring. Your only option to carry HD over coax is component and that's going to use 5 of your 6 cables (3 for video, 2 for audio) and you'll probably need a switch so you can hook up multiple sources to it and switch to the one you want to view on the TV.
Also, composite (single yellow connector) and s-video can not support HD, only component, DVI, HDMI and RGB can.
As for the flat panel over the fireplace, hope your not sitting too low, your TV should be mounted with the center at eye level, having it high can cause neck strain issues with any extended viewing. You should probably tilt it down a bit if possible when you mount it.
