Could be the amp or just some fuses. Mine did the same thing. After a round about long process, which I will not get into here for the sake of space, it turned out to be some fuses. Hope it all works out.
Sounds like the output transistors are breaking down under operation. In any case, the plate amp needs service, but it seems like the driver is ok. If servicing the plate amp is over $150, bail out and just buy a new one from Parts Express that closely matches yours in power and dimensions (or just buy a new sub...which might actually be cheaper).
