Extension usually refers to the lowest frequency it will reproduce and output refers to the maximum volume it will create. Both should be qualified with criteria used for the measurements.
I assume you are looking at HSU subs...

Max extension = the sub will play lower frequencies in this "mode"- i.e. if it normally goes down to about 20Hz, max extension mode might extend it to 16-17Hz. You sacrifice some SPL in this mode.

Max output = the sub is configured to pump out as many dB as possible across the frequency range. To accomplish this you sacrifice some low end extension to maximize SPL across the band.

So all in all you have two different setups depending on what you want...if you want the capability to play those REALLY LOW frequencies, you give up some SPL/loudness to do so. If you want maximum SPL and output, then you sacrifice some ability to get those lower frequencies.

In general,

Max ext= It is a setting on the sub that lets you play lower notes of the recording but not as loud as in max output.

Max Out =Setting in which you play all most common frequencies louder but it does not play the lowest registers of the recording...
