It's pretty common to use CAT 5 for phone now-a-days.

The CAT 5 has 4 pairs inside. One pair per phone line for most of us.

As far as using one CAT5 for network AND phone yes it's possible but not recommended, as a separate CAT 5 is always better.

It depends on the network topology. 10 Base-T (10 megabits) uses four wires, 100 Base-T (100 megabits) uses all eight wires. So if you use 10 Base-T, you can use the other four wires for phone. It is not recommended, but if you have no other choice you should be just fine. As far as speed goes 10 Base-T is still blazingly fast, so I wouldn't lose ANY sleep over it.
10/100 uses 2 pair

1 gig (new) uses all 4 pair.

David is correct, I misspoke. 100Base-T only requires 8 wires when category 3 is used (100 Base-T4), not when category 5 is used (100 BaseTX).
I have done exactly what was described above in my home for all runs with no issues except needing to teminate both ends to a RJ45 jack in a box so that I could make cleaner connections at each end with both a RJ45 and a RJ11 jack instead of having 2 plugs attached to the same cable with the jacket pulled back.

I am not sure how C Schelp did it but all I would do is terminate 4 wires on a category 5 jack and terminate the other 4 on a phone jack. Leviton makes a QuickPort system that you can find at most Home Depots that allows you to have both jacks on one wall plate. When I do it, I like to use a different color for each, so that it immediately obvious which is which. Even though you will have to pull the jacket back a little bit with my approach, just make sure you do not untwist the pairs going to the Cat 5 jack (you want to maintain the twist as far as possible).

I think C Schelp is saying he terminated all 8 into a Cat 5 jack and then split the signal at the wall.

I would guess Deans' answer will be something like this:

I can't support the death penalty under our current system but might support it under an improved system but I would have to see a better system in action first before I could say defintively.
