My HTPC is setup with:

Audio: M-Audio Audiophile 2496. It does S/PDIF out to your DACs at 44.1KHz, 48KHz, and 96KHz and allegedly has a good analog section. As a bonus, it won't output the 22.05 KHz windows clicks, bells, and beeps over the digital connection if you let the output rate float.

Video in: I/O Magic PVR. dscaler plays well ith it.

Video out: Radeon 7200. Excellent scaling (bicubic interpolation), good deinterlacing, iDCT off-load, and no fan.
