Where do the cable & small dish providers get their master HDTV feeds from?

I stumbled across a few websites on C-Band satellites and they say that the master copy / 1st generation broadcast from HBO, Cinemax, etc. is broadcast via C-Band and that is how the cable & small dish companies get their feed. They then re-compress it further (i.e. HD-Lite) and that is what gets transmitted to us. However, nothing was mentioned about where they get the local channels from in HD. Does anyone know where/how to get the master copy / 1st generation broadcast of the local channels?

Most providers pick up the broadcast over the air, and a number receive the same feed over fiber. A very small number of stations provide a better-than-OTA-quality fiber feed to local providers; the only stations I have read about doing this are PBS member station KQED in San Francisco (to Comcast) and NBC O&O WNBC in New York (to TWC).

I live near a large Charter cable outlet and they do have a huge dish. Whether they are using it or not I don't know.
