I have the PSU in the attached picture. I took the fan out and when I replaced it I forgot which way the airflow was directed. Should the fan be pushing air inside the PSU so that it flows out the vent holes in the back? Or should the fan be pulling the hot air and blowing it out of the PSU?

I think that it was setup the 2nd way, but I can't remember. Of course isn't the 2nd way kind of weird because then it is blowing hot air into my computer case...

I think the fan should be blowing hot air out back of the PSU(where the power connection is).

You want to pull air out of the case, so if the fan is on the outside it should push air out of the case. If the fan is inside it will push air into the psu then out the vent in the back.

edit - just looked at the picture again

looks like the fan is on the inside of the case so it would be the second thing i said, pull the air from the case INTO the psu then it vents out the back.
